KOZAK, William " Bill " Passed away peacefully with his family by his side on December 2, 2011, at the age of 87 years. Loving husband of Mary (O'mara) for 62 years. Beloved father of Dennis Kozak(1997), Sharon Kozak, Patti Kozak, Sandra and Mike Mitchell. Dear grandfather of Dave and Deb, Bettina, Brandy, Connie, Christine and Matt, Cindy, Brad and Anna, Sandy, Shelly and Andre". Great grandfather to Blake, Gavyn, Miranda, Makenzie, Savannah, Coryn and Sophia. Bill worked at Allstate Insurance for over 35 years.
